South Dakota Parent Connection

Your Input is Needed

The South Dakota Department of Health invites you to participate in the Maternal & Child Health Needs Assessment Survey to help them improve their services.

The South Dakota Department of Health Office of Child and Family Services asks for your help to gather information about the health needs of women, children, and youth including those with special health care needs, and their families.

What: Complete a short survey to help us understand and address the health needs of women, children, and youth including those with special health care needs, and their families.

Who: All South Dakotans

Why: The data being collected is not research, rather findings from the survey will inform long-term planning and programming for the Department of Health Office of Child and Family Services (OCFS) programs to better support the health needs of South Dakota women, children and youth including those with special health care needs and their families.

How: Please click here to take the survey.
