Food Insecurity Survey for Families with Children Who Have Disabilities 

Do you live in South Dakota and have a child(ren) in your household under the age of 18 with a physical, developmental, behavioral, or emotional condition, who needs services beyond that required by children generally? 

If so, you are invited to participate in a South Dakota research survey to measure food insecurity (not having a sufficient amount and/or variety of food to meet nutritional needs) among families who have children with disabilities. You must be age 18 or older to participate in the study.

This confidential, online survey includes questions about your household’s access to food, access to services, and your child(ren) with disabilities.  The survey should take about 20 minutes to complete and you can skip questions you prefer not to answer. Participants who complete the survey can choose to enter a raffle to win 1 of 4 gift cards valued at $50 each.

Families are encouraged to share this survey with other families via email. To protect the survey from bad actors, please do not post on social media.
