Disability Information Access Line (DIAL)

Help improve the new Disability Information Access Line (DIAL)!  DIAL is a free public service created to support people with disabilities and their families who are seeking information related to COVID-19 vaccines, local vaccination sites, and other local resources to help them live independently.

Please complete this short, anonymous questionnaire, which should take 5-10 minutes to complete. Your responses will help us better understand the experiences of people with disabilities during the COVID-19 pandemic, particularly experiences with accessing the COVID-19 vaccine.
